On December 9, 2003, the Office of Inspector General (OIG) was notified of the December 6, 2003 death of 16-year-old Cindy Gilbert, a female student at the Chemawa Indian School (CIS) in Salem, Oregon. Initially, BIA Internal Affairs Division (IAD) was assigned to conduct an administrative review of the incident while the FBI conducted the death investigation.
